Our mission is to improve the lives of cancer survivors by advocating for personalized, comprehensive high quality of care after cancer diagnosis and treatment. As survivors travel through the different phases of their cancer journey, Moffitt Cancer Center’s Survivorship Clinic can help you to address different challenges that emerge after completing therapy. We are committed to recognizing survivorship as a distinct phase of the cancer care continuum. There are four components of Survivorship Care we focus on are:
- Prevention of recurrent and new cancers, and of other late effects
- Surveillance for cancer spread, recurrence or second cancers, assessment of medical and psychosocial late effects
- Interventions to help with effects of cancer and its treatment
- Coordination between specialists, oncologists and primary care providers to ensure that all of the survivor's health needs are met
These four pillars guide the care we provide in our Survivorship Clinic. After you finish cancer treatment, our clinic will continue to follow you carefully for the surveillance of your primary cancer. In addition, we include other key information about your health and well-being as it impacts your overall wellness.
Our clinic has created a consultation service if your provider feels you would benefit from a visit with a Survivorship Provider. We offer a one-time Onco-wellness visit as patients are transitioning from Cancer therapy into surveillance. Within our clinic, we also created a focus on Adolescent and Young Adult (AYA) cancer survivors because we know the unique physical, psychological and social of cancer survivors under 40 years of age. For those survivors who have received cancer therapy that can affect the heart are followed more proactively by our Cardio-Oncology provider.
Our goal is to help you with a successful transition from cancer treatment to survivorship care. Upon completion, we provide you with a comprehensive survivorship care plan. The plan includes a full cancer treatment summary with the providers, cancer surgeries, pathology reports, chemotherapy and radiation therapy agents and doses, future surveillance schedules, cancer screenings, nutrition, dietary, activity recommendations, addressing fear of recurrence, long-term or late effects that you may have from your cancer or treatment. You can share the survivorship care plan with your primary care provider and any other specialists you may see.
